On the cuffs, one loop will end up on the inside of the inner seam. For spacing them, I start by marking them one inch to each side of the inner seam and spacing the other loops two inches apart on the outer side of the cuff. These measurements may need to be adjusted, depending how the cuff is fitted. For example, by the time I was done with John's cuff, the spacing between the buttonholes turned out to be one and seven eighths inches.
